I am aware of the risk of Covid-19 associated with participating in any in-person experience, including all Art Scōl Classes & Programs I will NOT attend this program mentioned herein if I am currently experiencing symptoms of fever, fatigue, difficulty breathing, or dry cough or exhibiting any other symptoms relating to COVID-19 or any communicable disease, or have experienced/exhibited any of these symptoms within 14 days of this event

I AGREE NOT TO ATTEND this program if I as participant or any minor participant(s) mentioned herein , or any member(s) of my household, have been diagnosed with COVID-19 within 14 days of this program.

I AGREE on behalf of myself as the participant or participants named herein to agree to hold harmless, release, waive and indemnify Art Scōl, its members, employees, agents, successors and assigns to any & all injuries, illness, medical conditions, claims, liabilities, damages, expenses or judgments from participation in any and all Art Scōl's programming and/or activities, including use of the area(s) and/or equipment, participation in a class or program at Art Scōl. Any defense, cost or expense arising out of any and all claims, injuries, liabilities or damages arising out of or related to participation in any and all of our programming, as set forth, including attorneys' fees.

I CERTIFY that I am responsible for any caregiver or guardian mentioned herein and guarantee that the all of the above is true and correct.

SNACKS, DRINKS, BELONGINGS *.Please do NOT share any of your belongings with any other student or member of staff. Please DO NOT SHARE any Food or Drinks with any other student or staff member.

Please Check * I understand that Art Scōl might take photographs or videos of art work or work in progress at the studio during classes which may/ or may not be used for publicity purposes. These photos may be published to our social media pages or platforms &/or website, &/or may be posted on other promotional materials or in other promotional media. NO STUDENT will ever be identified in these photographs, no student or student's work will ever be tagged or named/ identified in any photo without their consent. Please note we archive all our work that is created at our studio, so we take photos for these purposes also. After class, when a project is completed, sometimes we will gather everyone for a group photo, if you do not wish your child to be involved please state this clearly in the special notes section of this form. All photographs are the property of Art Scōl and we do not allow these photos to be published by third party or by any other media or source. We value & are protective of our students always and their interests & privacy.

CLASS START & END TIMES * All students are expected to arrive on time & all minor students must be picked up promptly. Classes start & end at the advertised times. If a student arrives late, they will NOT be given extra time at the end of class to finish their project. If a minor is not picked up on time extra charges apply after time & must be booked at time of registration.

RESCHEDULING * Rescheduling requests are NOT ACCEPTED by last minute phone or texts to any member of staff. A verbal notification to a teacher is NOT VALID, as teachers do not handle admin & will NOT BE ACCEPTED. If you do not show up for your scheduled class and do not notify us within the time frame mentioned above you will be marked as a no show & forfeit any right to rescheduling or credit. All rescheduling requests are subject to availability in our other classes and therefore NOT guaranteed .If your class is rescheduled it will be in another class with availability & during your current enrollment period. All rescheduling requests must be received by email to sdartscol@gmail.com at least 24 hrs prior to missing any scheduled session. Please see our studio polices page on our website
